We are enjoying the dog days of summer, being active and moving about this wonderful city.

Unfortunately, I recently saw a patient who isn’t moving as freely as she’d like. She broke her toe while helping an elderly woman into her wheelchair. The wheelchair ran over her foot. Ouch!

WHAT IS A FRACTURE?

A fracture is a broken bone that usually occurs from trauma and injuries.

TYPES OF FRACTURES

There are four types of fractures:

  • Stress fractures are small cracks on the surface of the bones caused by increases in physical activity, improper training techniques, changes in walking on uneven surfaces, and body changes in menopausal women who have a risk of osteoporosis.
  • Traumatic fracture is an injury to a bone caused by a variety of incidents, including falling objects, twisting injuries, or stubbing/jamming injuries.
  • Open fractures occur when bone punches through the skin and is exposed outside the body.
  • Closed Fractures occur when bone does not puncture through the skin and stays inside the body.

Fractures can also be classified in terms of their alignment. A non-displaced fracture means the bone has not moved from its original position. Displaced fractures are when the bone has moved out of alignment.

SYMPTOMS

Symptoms that occur are the following:

  • Pain
  • Bruising (black and blue)
  • Swelling
  • Difficulty walking (depending on location)

DIAGNOSIS

Fractures are diagnosed with a history and physical examination by your doctor. We will perform x-rays to look at the bone anatomy. A CT scan may be performed when there is a complex fracture. MRIs may also be performed to identify a stress fracture. MRIs can also evaluate pediatric fractures. Children have growth plates that allow their bones to grow. If there is an injury in this area, it cannot be detected by an x-ray alone.

TREATMENT

Fractures of the lesser toes (phalanges) occur when the foot hits a hard object or heavy object fall on the foot. If the toe is non-displaced (hasn’t moved), a cast is usually not need. We can splint your toes together for stability. We can use a post operative shoe for stability and comfort for several weeks. We will prescribe anti-inflammatory or pain medication as needed.

If the fracture is displaced (has move out of alignment), surgery may be needed. We will realign the bone via manipulation or surgically realign the bones with use of wires or screws. You will be in a post operative shoe or boot for several weeks.

Fractures of the big toe (hallux) tend to have more pain, swelling, or even bleeding than the lesser toes. It usually occurs when you stub your toe or a heavy objects fall on it. Treatment may require a walking boot or a cast if it’s non-displaced. If it’s displaced, the toe will either need manipulation or surgery to correct it.

If no treatment is given, there are several problems that can happen:

  • Arthritis at the joint
  • Chronic pain
  • Deformity
  • Lack of motion
